What is an autologous blood donation?

Updated 27 June 2014.

An autologous blood donation is when a person donates blood planned for their own use before elective surgery. There are at least three types of autologous procedure:

  • Preoperative autologous donation - patient gives his / her own blood before surgery. Preoperative autologous blood donation is the most common of the three methods.
  • Intraoperative recovery - an average reduction of blood loss during surgery, so that they are returned to the patient. It uses a machine called a computer screen.
  • Postoperative cell salvage - usually after the patient leaves the operating room. In some surgical procedures, it may be an accumulation of blood in the body is collected and can be transfused to the patient.

What is an autologous transfusion?

Donated blood to a patient prior to elective surgery and stores for use are registered. When blood is given to the patient as autologous blood transfusion.

Who decides whether to be a donor's own blood before surgery?

If there is a possibility that you need a blood transfusion during surgery, your doctor may recommend an autologous blood. The type of operation determines the need for autologous and health of the patient determines their ability to donate. You will need an appointment at the local blood bank, whether autologous blood donation.

Can you give a unit of autologous blood, if you ever been rejected as a blood donor?

Not necessarily stop rejection before you donate autologous unit. You will be tested as soon as you to get to your appointment, the blood bank. A short history of medicine is that you and your pulse, blood pressure, temperature and a blood sample from the fingertip, (to see if you have anemia review) to determine hematocrit. Your medications will be discussed, so make sure you arrive at your appointment with a list of your medications. (I was with my appointment as rheumatoid arthritis (RA) a person is not from a blood donor is usually not told. However, a patient with RA Methotrexate should last for 30 days until you can make a donation to the general population ). The requirement that methotrexate rises back of his own blood in order to be given with autologous blood.

Why go to the trouble of units of autologous blood donation? How to take advantage of autologous blood donation?

By donating one unit of autologous blood that:

  • Eliminate the risk of infectious disease and blood transfusion.
  • Although the blood of strangers, family and friends are all inspected and tested to minimize the risk of transmission of infectious diseases, autologous donation is the only way to eliminate the risk.
  • In addition, the use of his own blood, not to reduce the blood supply to the community and let people who can need.

Is there a cost associated with autologous blood donation?

Yes, there are costs associated with autologous blood. Check with your bank blood processing and administrative costs and check to see with your insurance if they cover their own blood.

An overview of the symptoms of tendinitis in ...

Updated 23 Mai 2014th

Written and reviewed by a Board Certified physician. See the Medical Review Board About.com.

What are tendons?

A chord is fixed a flexible fiber cable that connects muscle to bone. Tendons are those of the bands. While tendons connect muscles to bones, ligaments connect bones together at the joints. Tendons are capable of large loads, such as a steel cable. Both ligaments and tendons are as soft tissue.

What is tendinitis?

Tendinitis, tendonitis is sometimes spelled inflammation of one or more tendons.

What can cause tendonitis?

Tendinitis can be caused by an injury, but more often it is caused by repeated minor injuries in the affected area. Under certain conditions, such as rheumatoid arthritis, gout, psoriatic arthritis, thyroid disease or diabetes Tendinitis can occur without trauma.

Who can develop tendinitis?

People whose jobs require repetitive movements are more likely to develop tendinitis. Examples are people who are gardeners, carpenters, athletes, or typists are. People with poor posture or poor body mechanics may be an increased risk of developing tendinitis, also.

Tendinitis is most common in adults, especially at the age of 40 or more (tendons lose elasticity with age), but can develop at any tendinitis.

What are the symptoms of tendonitis?

Tendinitis causes pain and tenderness in the area just outside a joint. It may also be a loss of mobility in the joint close to the affected tendon. In most cases, shoulder, elbow, wrist, thumb, hip, knee and ankle joints are affected.

How is tendonitis diagnosed?

Most forms of tendonitis can be later diagnosed medical history and physical examination. Your doctor can perform special maneuvers using physical examination to determine which tendon is involved.

The test injection of anesthetic may be useful to confirm a diagnosis of tendinitis. A small amount of an anesthetic, such as lidocaine is injected into the affected area. If immediate relief of pain, which can confirm the diagnosis of tendinitis. MRI or ultrasound may also be used to search tendonitis and tears.

What is tendinitis?

The goal of treatment is to treat the inflammation. Rest, compression, elevation, and anti-inflammatory drugs are part of the initial treatment. Ultrasound, massage, gentle stretching and strengthening exercises, the use of protective equipment such as braces or splints can be used all the time. A steroid injection into the affected area is sometimes as if all else fails. A partially or completely torn tendon may require surgery.

Copper Hands: Past & quot; As Seen on TV & quot; Product ...

Updated September 22, 2014.

My curiosity is getting the best of me when I see a new product infomercial or "As Seen on TV" to relieve arthritis pain. Google I inevitably see the product, what it is and how it works. I hope my results, good or bad, to share with you or someone who has not heard of the product.

Claims

My latest discovery while watching TV late at night, was definitely copper. My first impression was that the concept combines the arthritis gloves with the supposed benefits of copper for arthritis. The August 18, 2014, I went to their website to learn more about the hands copper (www.copperhands.com). The information was to find what I had expected:

  • "Hands copper help arthritis, injuries and pain from repetitive movements that can occur in the hands and joints. It is also beneficial for stiffness and swelling."
  • "The gloves have to circulate a continuous compression in the area of pain on pressure, blood flow. Gire, pain in muscles and joints."

The gloves are made of copper hands spandex 90% cotton, 5% copper and 5%. Gloves have a design that you can resume your normal activities while wearing them. They can be worn all day and night.

The rest of the information seemed to the design and control of copper hands together. There have been no scientific studies substantiate the allegations copper compression and relieves arthritis pain.

What we know

Hands Copper is one of the many products Tele Brands, Inc. This is the same company that makes the Hurricane Mop, Peter Shaft, and pockets the tube, just to name a few. The website URL hand copper was recorded in January 2014, what. Upon a relatively new product I went back to September 20, 2014, to collect some more data on your website, and I found that is no longer online! It draws back telebrands.com hands where copper is not even mentioned!

Then I do a search online for copper hands and some critics appeared in the results. Copper hands Some people who have never ordered the gloves were given their orders and others have suggested, a "fraud". The few who have received your order complains that the gloves were impregnated with copper, but instead had copper-colored paint on the surface of the tissue in the form of points. I also checked his Facebook page and Twitter page. Both were removed.

We are often in products to make false claims or are actually unproven remedies maintained. This particular product has come and gone quickly. It may come back. If this is the case, please email me and I will arthritis@aboutguide.com this product update. If you still have problems with placing an order, learn, contact the Better Business Bureau for advice.

To understand Lyme disease, from start to finish ...

Updated 26 June 2014.

Treatment of Lyme disease

Use of antibiotics properly, your provider health care can effectively treat your Lyme disease. Generally, treatment begins earlier post-infection, a rapid and complete recovery.

  • Antibiotics such as doxycycline, amoxicillin or cefuroxime orally for a few weeks, can accelerate the healing of skin rash EM and usually avoid the following problems, such as arthritis or neurological symptoms.
  • Doxycycline also effectively treat most other tick-borne diseases.

Doxycycline can stain the permanent teeth developing in the womb, when the Lyme disease occurs in children under 9 years old women, or are pregnant or breast-feeding, which are usually treated with small children or babies:

  • Amoxicillin
  • Cefuroxime
  • Penicillin

Lyme arthritis

If you have Lyme arthritis, your doctor may be treated with oral antibiotics. If your arthritis is difficult you may ceftriaxone or penicillin are administered intravenously (into a vein). To relieve symptoms and promote healing, can also with your doctor:

  • prescribe medications such as NSAIDs (non-steroidal anti-inflammatory)
  • perform common aspiration (liquid design of affected joints)
  • surgical removal of the inflamed mucosa of the affected joints

For most people, Lyme arthritis will disappear in a few weeks or months after treatment with antibiotics. In some countries, it may take years to disappear completely. Some people with Lyme disease can be cured with appropriate antibiotics untreated for a number of years of the disease. If the disease persists long enough, but it can lead to permanent damage to the joint structure.

Neurological problems

If you have any neurological symptoms, the doctor will probably treat him with intravenous antibiotics ceftriaxone once daily for a month or so. Most people recover completely.

Heart problems

Health care providers prefer to treat people with Lyme disease with heart symptoms with antibiotics such as penicillin or ceftriaxone intravenously for about two weeks. Persons with Lyme disease often heart problems in the long term.

Research on Lyme Disease

After treatment of Lyme disease, you can still muscle pain, neurological symptoms such as problems with memory and concentration, and fatigue.

  • Researchers from the NIH-sponsored studies to determine the cause of these symptoms and the best way to treat.

Studies suggest that people who may suffer from chronic Lyme disease genetically predisposed to develop an autoimmune response that contributes to the symptoms.

  • Researchers are studying the importance of this finding in detail.

Researchers are also the best time to find give antibiotics to the different signs and symptoms of Lyme disease.

Lyme disease can recur

Unfortunately, a bout with Lyme disease to prevent the disease does not return. The disease may affect more than once, if it is infected by the bacteria of the Lyme disease.

Additional Resources

NIH conducts and supports biomedical research to meet the challenges of Lyme disease. Scientists to better understand the immune response to Lyme disease in humans.

  • For example, discover the mechanisms for the treatment-resistant Lyme arthritis responsible. A better understanding of the human immune response may lead to better tools for diagnosis and prognosis.
  • For example, B. burgdorferi immune complex assay, a test in development, suggesting active Lyme disease is not the antibody tests currently used infection. Since Lyme disease is difficult to diagnose and sometimes not respond to treatment, researchers are trying to protect a vaccine for humans against infection. Vaccines work, partly by.'s Body to produce antibodies These molecules are tailored to particular made by a bacterium or a virus blocking protein. Frequently these proteins are found in the outer shell of the bacteria. If the antibodies against the invading microbes to bind, called other immune defenses to destroy.

The key to progress

Although Lyme disease presents many challenges, these are the challenges of medical research is well equipped to react. New information on the Lyme disease accumulate at a rapid pace, performed all over the world thanks to the scientific research.

Understanding the causes and remedies for lower back ...

Updated June 3, 2014th

Back

The centerpiece is the spinal column that supports the weight of the upper body and houses and protects the spinal cord. Stacked more than 30 bones (vertebrae) that form the backbone or spine. Each of these bones has a circular opening, which, when stacked in alignment with each other to form a channel which surrounds the spinal cord.

Spinal cord

The spinal cord is from the base of the brain just below the rib cage. Small ribs (roots), and enter the spinal cord in the spaces between the vertebrae arise.

Horsetail

Because the bones of the spine continues to grow after the spinal cord reaches its full length in early childhood, the nerve roots in the lower back and legs extend many inches of the spine before leaving. This bundle of nerve roots called the cauda equina or "horse's tail".

Structures to

The spaces between the vertebrae include cartilage round mat called spongy discs, to permit flexibility in the lower back. Strips of tissue such as ligaments and tendons hold the vertebrae known on the spot and to the muscles of the spine stick.

Four regions

Starting at the top of the column has four areas:

The lumbar region of the back, where you will feel more pain, carries the weight of the upper body.

Cause of Restless Legs Syndrome

Updated May 20, 2014th

Restless legs syndrome is a rare disease whose cause is unknown. Studies have identified a possible cause of restless legs.

What is restless legs?

Restless legs syndrome (RLS) can be between 5 and 10 percent of the American population that is underdiagnosed influence. The main symptom is an irresistible urge the legs to move often accompanied by feelings legs "creepy crawlies".

Only the movement of the legs relieved feelings. For the millions of people who suffer the evening of restless leg syndrome worse after sunset, leading to a pattern of insomnia.

A team of researchers from Penn State College of Medicine and the Johns Hopkins University to search for answers restless legs. Hosted by James Connor, Ph.D., professor and interim chair of the Department of Anatomy and Neuoscience, Penn State College of Medicine, the team performed the first analysis of the autopsy of the brains of people with restless legs syndrome. Their research was presented at the meeting of the Associated Professional Sleep Societies in Chicago on June 5, 2003 showed a possible cause of restless legs.

The results of the study

  • Although no clear pathology in the brains of patients with restless legs syndrome appears that the cells in a part of the brain is not enough iron.
  • There were no signs of neurodegeneration, brain lost or damaged, as observed in Parkinson's disease and Alzheimer's disease cells.
  • The discovery of a natural cause of restless legs syndrome shows that the problem is psychological rather than sensorimotor.
  • Go as cells lost or damaged, but iron deficiency, there is hope for the development of treatments.

About the Study

  • Connor studied from the collection of the brain Restless Legs Syndrome Foundation Harvard Brain Bank received brain tissue.
  • Tissues were 7 people with Restless Legs Syndrome investigated. 5 samples from people without neurological diseases as controls were used.
  • Cross slide of the substantia nigra, allows the part of the brain, through the medium associated with the syndrome, the research team to the structure and function of cells in restless legs investigate.

Conclusions

  • Connor was found that a specific receptor for iron transport is lacking in patients with Restless Legs Syndrome. Enough iron occurs in the brain cells to keep them alive, but not enough to function optimally for them. It has been suggested that the lack of iron may lead to misfires of nerve signals to the legs cause uncomfortable tingling.
  • These results do not suggest that the person has an iron deficiency and require dietary supplements. It is suggested that the receptors to not specified as a packaging and delivery of a sufficient amount of iron, cells specific in the midbrain.
  • Some patients have time, take iron supplements found relief, but it is extremely important that any complementary therapy be administered by a physician.
  • Connor hopes to continue to detect other errors in the packaging of iron transport system in this part of the brain, as well as focusing on genes that regulate iron transport proteins.
  • The hope is that these findings are finally in a test to diagnose restless legs and treatments that might provide relief from restless leg syndrome long-term suffering.

This study was partially supported by grants from the National Institutes of Health and Restless Legs Syndrome Foundation.

Beta-cryptoxanthin in fruits and vegetables can ...

Updated on 13 May-2013.

Carotenoids Arthritis Risk Cut

Just drink a glass of orange juice a day, the risk of developing forms of inflammatory arthritis. Researchers found at the University of Manchester in Britain in the American Journal of Clinical Nutrition (August 2005) that certain carotenoids, including beta-cryptoxanthin and zeaxanthin reduce the risk of developing arthritis. However, two other carotenoids lutein and lycopene, did not appear to have the same protective effect against arthritis.

Study: Are fruits and vegetables reduce the risk of arthritis?

The researchers analyzed data from a study with 25,000 participants to determine whether carotenoids affected risk of arthritis. The study participants have a diet diary and were followed between 1993 and 2001, new cases of inflammatory polyarthritis, defined as synovitis (inflammation of the lining of the joint), to identify the two or more mixed groups.

The results of the study

Eighty-eight participants who developed arthritis, were found. They were compared with 176 healthy people as controls. Had 20% lower developed The average daily intake of zeaxanthin in the group arthritis patients than in controls. The average daily intake of 40% beta-cryptoxanthin was lower in patients who had developed arthritis in the control population.

The upper third of the intake of zeaxanthin and beta-cryptoxanthin less likely to develop inflammatory arthritis than that in the lower third of the inlet.

What is Beta-cryptoxanthin?

Beta-cryptoxanthin is classified as pro-vitamin A carotenoids. In the body can lead to an active form of vitamin A. Vitamin A is known to be important for the function of the immune system, and health of the skin and bone are converted. Beta-cryptoxanthin is found in fruits and yellow or orange vegetables. Here is a list of fruits and vegetables yellow and orange www.5aday.org:

  • Yellow Apples
  • Apricots
  • Cantaloupe
  • Uchuvas
  • Yellow figs
  • Grapefruit

  • Carrots
  • Yellow Bell Pepper
  • Yellow potatoes
  • Pumpkin
  • Turnip
  • Yellow Summer Squash
  • Corn
  • Sweet potatoes
  • Yellow Tomatoes
  • Yellow Winter Squash

What fruits and vegetables are rich in beta-cryptoxanthin?

According to NutritionData.com, a full list of the 467 richest beta-cryptoxanthin food shows:

  • Peppers, pumpkins, squash and vegetables with the highest levels of beta-cryptoxanthin per serving.
  • Persimmons, tangerines, papayas and fruit with the highest levels of beta-cryptoxanthin per serving.

What zeaxanthin?

Zeaxanthin is another carotenoid with antioxidant power. After www.5aday.org, food sources of zeaxanthin are green vegetables, fruits, yellow / orange and vegetables.

Research at the University of Manchester supports previous studies that have concluded that dietary antioxidants such as beta-cryptoxanthin and zeaxanthin as well as vitamin C may be associated with a lower risk of arthritis.

Determining the cause of anemia is crucial ...

Updated May 26, 2014.

Written and reviewed by a Board Certified physician. See the Medical Review Board About.com.

Anemia is a condition where the blood has an unusually low number of red blood cells or amount of the hemoglobin occurs. Hemoglobin is the iron-rich protein binds the oxygen in the lungs, so that he transported to the tissues of the body.

Anemia in patients with inflammatory arthritis such as rheumatoid arthritis together. For example, anemia of chronic diseases is a specific type of anemia which develops in response to inflammation. Anemia of chronic disease is, however, distinguished from other types of anemia, because the treatment depends on the type.

The symptoms of anemia

The most common symptoms are fatigue, anemia, shortness of breath, dizziness, palpitations, cardiac arrhythmias, headache, cold hands, cold feet, pale or yellowed skin, and chest pain. A person with anemia may include one or more of these symptoms occur. If there are no obvious signs or symptoms of anemia, the disease may go unnoticed until a blood test done.

Types of anemia

Iron deficiency anemia is the most common form of anemia. As the name implies, this type of anemia develops when you miss a sufficient amount of iron in the body. In general, the blood loss is a result of iron deficiency anemia, the poor absorption of iron but may also cause disease.

Vitamin deficiency anemia may develop if there is a low level of vitamin B12 or folic acid in the body. With vitamin B12, vitamin often not well absorbed. Pernicious anemia is one of many causes of vitamin B12 deficiency.

Aplastic anemia is a rare type of anemia when the body is not developed enough to produce red blood cells. Viral infections, exposure to toxic chemicals, autoimmune diseases, and some medications are considered as possible causes into consideration.

Hemolytic anemia occurs when an abnormal distribution of red blood cells in the blood or spleen. Possible causes are mechanical reasons (eg aneurysms), infection, autoimmune disease or congenital or hereditary disorders (eg, sickle cell anemia).

Anemia of chronic disease is a disease that develops anemia secondary to another disease. Can cancer, kidney disease, liver disease, thyroid disease, rheumatoid arthritis or condition that interferes be associated with the production of red blood cells.

Distinguish the anemia of chronic disease iron deficiency anemia

For people with inflammatory arthritis, it is important to distinguish between the two most common types of anemia that affect them - iron deficiency and anemia of chronic disease. Many arthritis patients taking NSAIDs (nonsteroidal anti-inflammatory drugs) as part of their treatment. NSAIDs are associated with an increased risk of gastrointestinal bleeding. Patients and physicians should be aware of the risks, symptoms and regular blood tests to check blood counts monitored. As mentioned above, the loss of blood to the underlying cause of iron deficiency anemia can be.

The anemia of chronic disease is altered iron metabolism. If the inflammation is triggered by the immune system, the metabolism of iron in the body goes into defense mode, so to speak. When this happens, there is a slight decrease in hemoglobin is less iron is absorbed by the body free iron in the body is stored in the liver cells, and the amount of increase in serum ferritin.

Anemia of chronic disease is not progressing. In general, the hemoglobin values rise somewhat lower than normal, usually not in love with 9.5 mg / dl. As iron deficiency anemia and anemia of chronic disease, serum iron is small. Small red blood cells can be seen under the microscope, or by state, but are usually of iron deficiency anemia. Transferrin, a protein that transports iron is high in iron deficiency anemia - a sign that the body is more iron. The total iron binding capacity (CBC), an indirect measure of transferrin, low in anemia of chronic disease - that it is not easy to get a sign, not enough iron, but. CTF is usually high when the reduced iron stores and low when iron stores are high. In iron deficiency anemia is usually greater than CTF 400 mcg / dL for iron stores are low.

Serum ferritin is often used to distinguish between the two types of anemia, but be higher in the presence of inflammation. With an inflammatory condition serum ferritin can be increased to a normal level, although iron deficiency. It can be confusing. The serum transferrin receptor test can help to solve this problem, as it is less affected by inflammation. In iron deficiency anemia, the serum transferrin receptor. In anemia of chronic disease, serum transferrin receptor, the lower side of the normal, or usually less.

Anemia of chronic disease is not treated with iron supplements. Extra iron can be harmful even after the underlying chronic disease. However, iron supplements can be specified in iron deficiency anemia. Must also, in the case of bleeding are the source of bleeding.

Which patients with arthritis need to know ...

Updated 27 June 2014.

Written and reviewed by a Board Certified physician. See the Medical Review Board About.com.

Chest pain is not an uncommon symptom. If it does not have to be rare, it is disturbing to say the least. Faced with chest pain, people often think of a heart attack. But some other conditions with chest pain also. It is imperative to be evaluated immediately and determine the cause.

In addition to heart problems, chest pain can be caused by a lung disease or gastrointestinal tract. Pain may also radiate to the chest with the disease of the cervical or thoracic spine. Musculoskeletal diseases and costochondritis Tietze's syndrome are also associated with pain in the chest wall.

Distinction between Tietze's syndrome and costochondritis

While costochondritis and Tietze syndrome are often seen as different names for the same problem, there is a feature that distinguishes them. Tietze's syndrome and costochondritis are caused by inflammation of the joints or nerves costochondral joints chondrosternal anterior chest wall. Connecting the sternum cartilage (ie breastbone), and the ends of the ribs - Both conditions are a speed of rib cartilage. However, there are local swelling with Tietze's syndrome and costochondritis without swelling.

Tietze's syndrome:

  • It is less common than costochondritis
  • Onset may be gradual or sudden
  • Swelling generally occurs at the second or third rib cartilage
  • Exacerbated the pain in the shoulder and radiates through coughing, sneezing or movement of the chest wall
  • Tenderness is felt
  • a website is costal cartilages involved in 70% of cases

Costochondritis:

  • More often, Tietze's syndrome
  • is associated with pain and tenderness of the chest wall, without swelling
  • Sensitivity usually includes an area costochondral in 90% of cases
  • the second to fifth costochondral junctions are often involved
  • It is also known as the front wall of the syndrome, syndrome costosternal parasternal chest syndrome chondrodynia

The diagnosis of costochondritis

During a physical examination in chest pain can be reproduced by palpation of the costal cartilage is usually sufficient to make the diagnosis of costochondritis in children, adolescents and young adults. In patients over 35, patients at risk or with a history of coronary heart disease, or anyone with cardiopulmonary symptoms are usually recommended in addition to the physical examination, electrocardiogram (ECG) and chest x-rays.

Costochondritis treatment

No clinical trials for the treatment of costochondritis. Treatment focuses on pain relief. Paracetamol, non-steroidal anti-inflammatory drugs (NSAIDs) and other pain killers are prescribed to relieve pain with costochondritis. With prescription drugs, pain management may rest, hot packs, heating pad, and avoid activities that increase pain. Physical therapy is often required, but in some cases can be a useful treatment option. Lidocaine / corticosteroid injections may be considered costochondral in the affected areas, especially when other treatment options have little or no relief, but rarely necessary.

What to expect

The time varies costochondritis. The condition usually lasts a few weeks. It can take months. Almost always costochondritis is set in the year. It is possible, though rare, to a persistent cases of chest wall have pain with costochondritis.

The Bottom Line

Patients with rheumatoid arthritis have a higher risk than the general population of heart disease. For these patients, particularly chest pain fear is natural. If you experience chest pain, should be evaluated immediately, can not be overstated.

Updated September 11, 2014.

When I experienced the first symptoms of arthritis, there are many who do not know what was going on and why I suddenly a swollen and painful knee. I waited a week or two to go, but if it does not, I went to my family doctor. Only 19 at the time it was.

My doctor was a bit perplexed. Since I was athletic and played tennis, the doctor ordered preliminary tests and referred me to an orthopedic surgeon. As suspected a family doctor and specialist in orthopedics had a sports injury. There was the possibility of arthritis, but I was so young and my test came back negative rheumatoid factor. It does not seem to be improving, so I searched and found that doctors who specialize in arthritis and other rheumatic diseases. The specialty called rheumatology and doctors who specialize rheumatologists.

Personal history, I have not shared, 1974-1975 Internet is available to me at the moment, so that you can access information and advice. My doctors seemed "too young You're" and trapped in these false ideas like "your blood test is negative." Finally, I asked for a referral to a rheumatologist. Long story short, here is where I belonged all the time. My rheumatologist could diagnose me with RA to recommend a treatment plan, and set me on the path to better disease management.

Early diagnosis and early treatment are the target

Over the years, the importance of consultation with a rheumatologist has become increasingly clear that early diagnosis and early treatment of arthritis have become prime targets for physicians. Typical approaches in the 1970s, a conservative treatment were exchanged for more aggressive therapies. An opportunity was found, suggesting that early and aggressive treatment for the disease is the best way to slow the progression of the disease. This applies especially to the types of inflammatory arthritis such as rheumatoid arthritis.

Possible long wait for initial consultation

Because of the new emphasis on early diagnosis and treatment, people with symptoms of arthritis are, or pointing to one of the rheumatic diseases usually accelerated to a rheumatologist for evaluation. But there is a problem to be an appointment. In some parts of the United States, there is a shortage of rheumatologists. Some rheumatologists have a large area and "too many patients, too few rheumatologists dilemma 'may cause delays in the appointment.

According to the August 2014 rheumatologists, a division of rheumatology in particular, Chattanooga, Tennessee can not see an urgent note in about 6 weeks. You see the most urgent recommendations within 1-2 days. But that's another story on the opposite side of the state, 300 miles west of Memphis, where the wait time of six months. There are more rheumatologists to Chattanooga from Memphis, Memphis, but is much higher. It's not always the size of the city - medium-sized cities in Tennessee have rheumatologists. And there are not only differences within a given state, there are differences across the country. For example, the northern states have more access to rheumatologists that the Western states.

In 2005, the American College of Rheumatology published a study that projects be for rheumatologists in 2020 around 6500 the demand, but only 5000 active exercise rheumatologists. In 2025, it was expected that the gap could grow to 2000 In addition to the question of regional availability of rheumatologists, there is the problem of rheumatologists in retirement are graduates to take their place. According to the rheumatologist, more than half of the existing batch rheumatologists are more than 50 years, the population is growing and at the same time, many rheumatologists will retire.

Rheumatologists are experts in arthritis and rheumatic diseases

Although it may be difficult for some people to see a rheumatologist because of distance or delays, here are the reasons why it is important to continue:

  • Rheumatologists are trained and highly qualified to make a difference in autoimmune diseases and rheumatic diagnosis (ie, to determine which type of arthritis or rheumatic disease that has).
  • Rheumatologists choose the appropriate medical treatment according to the diagnosis and consider comorbidities and the patient's lifestyle.
  • Rheumatologists are trained and experienced in order to monitor the effectiveness of treatment and recognition of the undesirable side effects of treatment.
  • Rheumatologists are focused on maintaining or improving the quality of life of patients and the prevention of disability in the provision of a management plan for rheumatic diseases.
  • Rheumatologists are qualified with the common claim and intra-articular injections, and the interpretation of imaging studies and laboratory findings for the diagnosis and monitoring of disease activity.

Who should be fired?

According to the American College of Rheumatology, the patient should be referred to a rheumatologist if you suspect rheumatic disease, but the diagnosis is not clear (eg patients who unexplained symptoms or the normal blood tests, the abnormal symptoms). Patients can also be returned with the results of abnormal laboratory test that can probably be associated with a rheumatic disease.

Patients have to deal with suspected rheumatic disease both confirm the diagnosis and long-term management. A rheumatologist may also be a second opinion from another doctor.

Regular use of aspirin-related macular ...

Updated January 21, 2013.

The results of a study in Australia show that regular aspirin may be published with a high risk of neovascular macular degeneration associated with age according to a report Jan. 21, 2013, be brought in JAMA Internal Medicine in conjunction. In the study, one or more times was per week as a regular application. Older people who use aspirin had twice the risk of eye diseases regularly.

A few years ago, aspirin was the drug of choice for people with arthritis. I was prescribed aspirin in the diagnosis of rheumatoid arthritis in 1974, day 8, have developed many new drugs and marketed for arthritis, including many other NSAIDs (non-steroidal anti-inflammatory drugs) are.

Some people take to prevent a daily dose of low-dose aspirin for heart attack. Some studies have suggested that low doses of aspirin may be beneficial for people with certain types of cancer, too. Although the link between aspirin and neovascular macular degeneration related to age is a problem, experts say, that prospective randomized studies should be performed to change before advising patients on their current use of aspirin. In other words, do not panic. Talk to your doctor.

10 ways to help a person with arthritis

Updated June 6, 2014th

Written and reviewed by a Board Certified physician. See the Medical Review Board About.com.

When you consider that people have different personalities, different levels of disease severity, and different needs, it requires making some thoughts on how to best help someone with arthritis. Needless to say, with greater severity of arthritis, there is an increased need for support. However, they are often people who help, because more than anything question will not be a problem, are reluctant. Here are 10 tips on how to help someone with arthritis, and I hope it will bring some ideas.

1 Sea loyal support of a loving friend or family member. Know the audience. Whether they can trust in their most intimate feelings. Be the person they recognize that they will never be alone.

2 -. Offer for lunch or dinner if you cook your own meals in the kitchen, plan to do the cleaning. Do not leave a mess! If it's easier, you can prepare your meals in the kitchen, make-do, or something delivered. Like all pull irrelevant. Just know a prepared meal is both useful and a gift for someone with arthritis.

3 - Gain a basic understanding of arthritis, so if your friend or loved one to share what has happened or how they feel, it is understood. Conversation with someone who understands is a great help. Conversation with someone who has taken the time to try to understand, is a blessing.

4 - Avoid talking about unproven remedies also do not offer medical advice of any kind .. Let cockroaches for professionals. Even with the best intentions, unsolicited advice about the area, which should be reserved for a doctor and his patient.

5 - Anticipate what your friend or loved one have when shopping for you, call and ask if you want to give something for them .. Or, you can invite them to go on the outside of the house and allows them to understand what they need.

6 - Make it clear that you are willing and able to help them know that they only ask for help .. Make it clear that you are willing to help, and they should not wait until they for what they need, are asking. Thy will must be as fact.

7 - Observe the area for things that might look specifically heavy tasks or things for, difficulties carrying out their own .. can in most cases this is tasks that require lifting or reaching, such as participating hanging pictures have, change the cat litter, the baskets laden clothing.

8 -. Encourage positive measures that could benefit their well-being, I'm not saying you will make your life coach. In this case is probably the best subtle. Instead of preaching the benefits of exercise, ask your friend or loved one if they wish to participate in a walk. Bring a basket of fruit. Buy a shake. They lend a book that inspires.

9 - Learn injected with inflammatory arthritis treated with injectable drugs such as giving some patients .. Some people hate to get a shot, but the auto-injectors are fairly simple. If your friend or loved one hates the process, and when you're ready, ready for the task you are to be considered unpleasant.

10 -. Offer if drive and if you are willing to give your friend or loved one go where they need to go. It can be helpful - especially if you do not feel well enough to drive themselves. Even if you do not feel bad, it's nice to sit back, relax and enjoy the ride to insist instead on the road. When you reach your destination, place it near the door, too!

The Bottom Line

Learning if difficult balance to step in and help when they resign. You probably do not go perfectly. Start slowly until you feel accepted, your support is and welcomed. And remember, do not drown!

4 words should & # 039; t tell people in ...

Updated September 15, 2014.

The preface

In some of my articles, I end up with a section I call "The Bottom Line". Emphasizes the point of the article. Here I am going to start with a "Foreword", because I believe it is important to get the message at first.

Some topics are sensitive. Some people are sensitive. No hypersensitive - just a little sensitive. This is to cope with difficult situations. This is normal. For example, illness, pain, loss and abuse are just some of the sticky wickets that can increase our sensitivity.

If you had one of these things first hand, you probably know that when people come to you, even with the best intentions, can something be wrong to say. The thing "thing" often wrong, but the last point here is well intentioned people -.... not offend you try anything wrong to say, just not what I say or what you prefer to hear.

I also do

I'm not about to do it yourself. Last year, died unexpectedly one of my dearest friends. I kept in touch with his daughter on Facebook. When my friend's daughter expresses deep sorrow on Facebook posts, I tend to position expressed as the pain in my own experience was for me. On second thought, I know I'm not always say the right thing. Pain is very personal. You can not, nor its planned course.

My best advice is for people to manage their words through a filter in the head and test. Would you like embarrassing what they say? But it's hard to. On the one hand, we have no time to send word through a filter before starting on his mouth most of the time. Second, the world is not wrinkled or bent by the same word or words.

It is also true that, if we knew better, better. When we use the words that the potential to transform a smile with a frown to learn to avoid them. It is only a matter of steering clear of some words that currently can kill some joy for some people, or some hair.

To avoid words

I asked a group of people with arthritis in their opinion about the words cock-tornadoes are possible. The consensus was that these four words have the potential to cause a negative reaction:

  • Disabled person
  • paralyzed
  • Disabled person
  • Victim

At this point you may be thinking: "Hey, those are just words." You might think that people with arthritis suffer from pain. Have physical limitations. They can do what they want to do. Arthritis can make building and are disabled. You are not wrong. But people with arthritis summon all the strength and courage that they did not feel like a victim, do not feel hurt not feel handicapped, disabled, or otherwise. Living with arthritis is normal.

Adapt people with arthritis to live with a chronic illness. They make the necessary adjustments to maintain a normal operation to continue, as far as possible. Accept arthritis as part of their lives, but do not feel that the disease define who they are.

This is not the end of the world if you, the right thing to say for someone living with arthritis or any other person who is facing difficulties. People are forgiven, after all. But maybe we can all do more. We can know a little more what we say. Maybe if someone you have arthritis, can transfer a call to the language that they prefer to be clarified. If you have arthritis, to talk and share their feelings. We should not expect people to know them. You have not spent our shoes.

If arthritic Methotrexa fear ...

Updated 30 June 2014.

Written and reviewed by a Board Certified physician. See the Medical Review Board About.com.

Some patients with rheumatoid arthritis were alarmed when methotrexate was prescribed. The drug is known to be a chemotherapy drug used to treat cancer be. But it is right to classify or consider a chemotherapy drug methotrexate in rheumatoid arthritis prescribed? Maybe seems a topic that absolutely anything, but when a misunderstanding caused unnecessary fear or caution in patients with a drug that can help them, the question is not really important. Consider the facts.

The short answer

There are more than 50 years, methotrexate has been touted as a cure for cancer because of its anti-proliferative effect. In the last 25 years, the drug was widely considered anti-rheumatic diseases (FAME) to treat rheumatoid arthritis and other rheumatic diseases. Methotrexate actually considered a standard treatment and the drug of choice in the treatment of rheumatoid arthritis.

After a chapter in the book DMARDs, rheumatoid arthritis: early diagnosis and treatment, "His popularity is due to the ease of use (subcutaneously or orally once weekly dosing) based toxicities are well defined, the rapid onset of clinical benefit (usually within 6-8 weeks), long life, low cost, additive benefit when combined with other treatments DMARDs, and its ability to radiological lesions retard. "

How does methotrexate

This is supposedly toxicities with the inhibition of dihydrofolate reductase and folate associated production. When used as background therapy, however, seem related a known anti-inflammatory mediator adenosine induction methotrexate benefits. To improve the effect of methotrexate on the production of folic acid, a folic acid supplement daily is the usual pattern for patients with rheumatic diseases.

When used as a chemotherapeutic agent, to prevent cells with folic acid, methotrexate, DNA and RNA, which are at slowing the growth of cancer cells.

Reduce the need for primary therapy

When used as a chemotherapeutic agent methotrexate administered in a moderate to high doses. The dose is determined by the size of the patient, wherein the cancer treated and kidney function. Greater than or equal to 500 mg / m 2 dose is as high, with doses of 50 to 500 mg / m 2 are intermediate. Low dose will be less than 50 mg / m2.

If methotrexate for the treatment of rheumatoid arthritis, the dose is estimated to be low - usually 7.5 to 10 mg / week. If necessary, the dose may be escalated to a maximum of 25 mg / week.

If side effects or toxicity develops, it is estimated that reducing the dose or switch to oral methotrexate injection can treat questions. Routine blood tests are also in patients to detect early methotrexate (complete blood count, liver and renal profiles) problems so that adjustments can be made to be performed. While rare serious side effects develop, discontinuation of methotrexate therapy is necessary.

The Bottom Line

When he was asked by a comment if methotrexate is to be feared, because it is used as chemotherapy, rheumatologist Scott J. Zashin, MD (www.scottzashinmd.com/) replied: "Although methotrexate High doses can for chemotherapy, rheumatologists used be with methotrexate due to its low anti-inflammatory doses. not uncommon for a pharmacist to make a possible drug interaction between NSAIDs (a common treatment for rheumatoid arthritis) and methotrexate, but not seen as clinically significant interaction with the low dose of drug, the for the treatment of rheumatoid arthritis. Patients should not forget, also, that the drug is prescribed only met once a week in order to minimize possible side effects. "

10 Signs of unproven remedies for arthritis

Updated June 1, 2014.

Wait and examine

The pain of chronic arthritis can be so painful that the person is willing to do anything to try to end the agony. The willingness to try anything lead a person to explore:

  • unproven remedies
  • Patent Recipes
  • other gadgets

In desperation, caution may be neglected, so that time and money to spend on arthritis remedies that have no value. Before a new drug or treatment of arthritis have been treated was examined.

10 signs of arthritis unproven remedies

The list of the ten routines is typical of promoters of unproven arthritis treatment products and remedies. The tricks used must be careful of your application.

# 1 - This is a "cure" Often the cure or treatment is a cure for arthritis. There is no known cure for chronic rheumatic diseases. If there is a real cure will be in the new world title.

# 2 - This is a "secret" Often the remedy is described as special or exclusive secret formula. Scientists are to hold no secrets or proprietary legitimate results.

# 3 - "witness", often presented the testimony of people who were allegedly helped by the remedy of arthritis as anecdotal evidence of its value.

# 10 - The "conspiracy theory" Often, various charity organizations and the medical establishment of "conspiracy" accused to stop its progress by not approve or support the healing of arthritis promoted. These often include organizations such as:

  • The Arthritis Foundation
  • The American Medical Association
  • NIH
  • The Food and Drug Administration

The best defense

Education and the responsibility for their own health care is the first line of defense against the promoters of charlatans, unproven treatments. Knowing the symptoms, new treatments and the reasons for the advice of health leads to intelligent questions and prohibits a person easily deceived.

Three federal agencies have the authority to take against the promoters of ineffective treatments.

  • The Food and Drug Administration may go to court to prevent anyone who sells a medicine for arthritis that has not been properly tested by them and approved for sale.
  • The Federal Trade Commission, any person or company that develops to the false advertising of a product for the treatment of a disease should be prohibited.
  • The post office in the United States has the power to take any letter to any promoter to sell a product of this type mail.

These organizations often do not have the staff to be fully effective, however.

The Bottom Line

Realistic hope is the necessary ingredient to avoid the temptation to use unproven arthritis. A proven cure for arthritis offers only false hope. The aim is someone who feels hopeless.

Before making a decision, consisted of seven men and five women for 10 hours over two days and a half advised the jury. The jury announced the price as follows:

  • $ 450,000 for economic damages
  • Anquish $ 24 million loss and mental Pet
  • $ 229 million in punitive damages

The applicant would be sought 40400000 dollars in damages. According to MSNBC, the amount should be reduced to punitive damages under Texas law limits the state - limiting punitive damages double the amount of economic damages awarded (lost wages) and $ 750,000 over the amount of non-economic damages (loss of fear and companies). The applicant has the maximum in this case, after the reduction is in accordance with the law would be $ 1.65 million in punitive damages to be made of the total $ 26.1 million damage, which is not exceeded.

The process

Ernst died in 2001 of a heart attack. The paper analyzes the cause of the fatal heart attack. Was that Vioxx caused the heart attack or arrhythmia was secondary clogged arteries, as described in the autopsy report?

The pathologist who performed the autopsy said Ernst likely to cause fatal heart arrhythmias and heart attack later in the trial of a blood clot. The pathologist could not 100% certainty that it was a blood clot (not found at autopsy) or heart attack. The jury verdict shows that he believed that the theory that caused a blood clot was cardiac arrhythmia and Merck Vioxx and responsible.

The eyes of the nation focused on the first trial of Vioxx.

  • What was the verdict sends message to all drug manufacturers and the administration of the United States Food and Drug?
  • What would be the point of the 20 million people who used Vioxx before September 30, 2004 recall
  • What impact would the judgment, if in the 4200 Vioxx lawsuit out of retirement?

Vioxx test

Not all Vioxx claims are wrongful death suits. The plaintiffs in the Vioxx trial starting next September 12 in New Jersey, is a man who has survived a heart attack in 2001, but the blame for Vioxx provoked the attack. The first federal Vioxx trial to begin until November 28.

Analysts suggest that if Merck continues to lose more requirements Vioxx, Vioxx study can multiply number exponentially. Analysts have also pointed out, Merck forced Vioxx trial rather than down they fight, one after the other, if the trend is that Merck lost. For now, it is not speculation, but the fact is that Ernst Merck will appeal the decision.

The problem with Vioxx

Vioxx was launched for the treatment of arthritis in the United States in 1999 and has been in 80 countries, marketed and used worldwide by 84 million people.

The decision to withdraw Vioxx, followed by a three-year study, as APPROVe (Adenomatous Polyp Prevention on Vioxx), which was a study to determine the effect of Vioxx on the recurrence of neoplastic polyps of the large bowel in patients known with a history of colorectal adenomas . The study found that the increased risk of cardiovascular complications began 18 months after patients started taking Vioxx.

Merck was to hide data sent warning signals on heart disease risk to have.

The COX-2

Vioxx was the second in a promising new class of drugs called COX-2. The first class Celebrex was approved by the FDA in 1998 and is still sold. The third of the COX-2, Bextra was provided by the FDA in 2001 due to lack of sufficient data on the cardiovascular safety of long term use of Bextra, an increased risk of adverse cardiovascular events in and set in April 2005 coronary artery bypass testing and coverage at short notice for severe and potentially fatal skin.

The Fallout

The recall of Vioxx caused concern for the entire class of COX-2 and caused the FDA come under observation, as well as older arthritis drugs called NSAIDs (nonsteroidal anti-inflammatory drugs). NSAIDs are commonly prescribed to reduce inflammation and relieve pain.

When the COX-2 were developed, they were classified as having gastro-intestinal profile was marketed safer. The popularity of the COX-2 greatly increased, ie, as the cardiac risk has become a problem.

The FDA changed the label of Celebrex and other NSAIDs, contain additional warnings. Like Vioxx, legal issues still to play.

Inaccurate information for continuing with arthritis and Propagation:

  • Arthritis is a disease of the elderly.
  • Arthritis is caused by a cold, wet climate.
  • Arthritis can be cured.
  • Arthritis is caused by poor diet.
  • Arthritis consists only mild pain.
  • "It felt good yesterday .... why so tired today?"
  • "You have arthritis, you can not ......"

There are many misconceptions about arthritis. The most common myths about arthritis seem to exist. Patients need data and valuable information, not myths and misunderstandings, so that they better manage their disease.

Myth # 1 - Arthritis is a disease of the elderly

Fact: Anyone can suffer from arthritis.

The most common misconception about arthritis is that it is a disease of the elderly. In fact, arthritis can affect anyone at any age, not only for seniors, including:

  • Children
  • Young Adults
  • Middle-aged people

Arthritis is not age or gender. There are over 100 different types of arthritis and related rheumatic diseases and some are found more frequently in certain groups.

Myth # 2 - Arthritis is caused by a cold, damp weather

Fact: The climate itself is neither the cause nor the cure.

It has been theorized for some time that arthritis is caused by a cold, wet climate. The move to a warm, dry climate was considered by some as a cure. Logically follows that if a cure arthritis warm weather, then no one is in Southern California or other warmer areas have arthritis. Bone rubs on bone after cartilage causing pain is to be worn in any weather. But:

  • Heat can be a relief.
  • Those who have arthritis often feel better in warmer climates.
  • Heat can relieve the symptoms of arthritis, such as immersion in a hot tub or take a hot shower.

Note: Since the move from Ohio to Southern California and Nevada, I can say that the warm weather has my minor aches and pains that accompany arthritic diseases reduced. I think one of the reasons due to the fact that I is more active and able, in this climate. Fear of falling on ice and snow is not a problem. I am able to run a lot more and I think the exercise has helped me tremendously. However, it did not the damage done to reverse my cartilage and joints to the disease.

Myth # 3 - The arthritis can be cured.

Fact: There is no scientific proof that a cure for arthritis exists today.

To date there is no known cure for the disease. Much has been discovered in terms of better treatment options and the progression of the disease, but nothing has been found to stop the disease successfully. Since there are so many different types of arthritis, the prognosis varies.

Since arthritis is an ongoing process, the importance of acquiring knowledge and understanding of their own health can not be overstated. There is much to learn:

Myth # 4 - arthritis is caused by poor diet.

Fact: There is no scientific evidence that foods cause or prevent arthritis-specific.

There was a lot of speculation about the importance of nutrition in relation to arthritis. Certainly, a balanced and nutritious and maintaining weight loss, improved nutrition overall health and well-being for all. There are some cases in which there is a clear connection nutrition (for example, sphere).

There is no scientific evidence that certain foods cause or prevent arthritis. Good nutrition does not prevent that arthritis. Unless a person with a food allergy, the particular flare up causing arthritis, there is no direct link between a specific food source and arthritis shown.

Myth # 5 - Arthritis is only slight pain.

Fact: Arthritis is much more than just mild pain.

There is another misconception that arthritis is simply connected with mild pain. TV ads, claiming that the couple of aspirin or other painkillers over-the-counter pain relieve pain and arthritis pain tend to confuse the public.

The display, with a lack of knowledge about the disease, expand the consciousness of some people the more complex forms of arthritis, the more aggressive forms of treatment.

The changes that can occur when a person of arthritis are affected:

Myth # 6 - "She felt good yesterday .... why so tired today?"

In fact: There is a change in the duration and severity of the symptoms of arthritis.

Since arthritis is a disease that is characterized by periods of remission and relapse, it is often difficult for someone with arthritis to understand friends and family why they feel much better or worse than any given day. The inconsistency of arthritis can even cause to believe some people that the disease "in his head."

Arthritis is characterized by a mix of good and bad days. Some days, joint pain and fatigue worse. A balance between rest and activity may be necessary to better manage living with arthritis.

Myth # 7 - "you have arthritis, you can not ...."

Reality: There is much more to do as a person with arthritis.

The limitations that imposes individual arthritis, the people that bring them close to overprotective. Sometimes people are just to try to help people with arthritis. The disease can interfere with physical ability, but certainly the arthritic person should not be as totally disabled and dependent.

A certain amount of aid and dependence is likely to be required. However, we must remember that it is preferable to keep the greatest possible autonomy for physical and emotional reasons.

Another type, the so-called inclusion body myositis is considered by some as idiopathic inflammatory myopathy to.

Inflammatory myopathies can exist alone or secondary to other rheumatic disease. Rheumatic diseases are often associated with inflammatory myopathies, including scleroderma, mixed connective tissue disease, Sjogren's syndrome, systemic lupus erythematosus and. Inflammatory myopathy can coexist with rheumatoid arthritis, as well.

The main symptoms of polymyositis and dermatomyositis are associated muscle weakness and low muscle strength. The weakness typically occurs symmetrically in the neck muscles, pelvis, thigh and shoulder. If left untreated progresses muscle weakness and, in severe cases, the mobility of patients may need help - perhaps even a wheelchair. When the contractility of the muscles of the neck are damaged, it may be trouble swallowing and nutrition.
